KEYGEN should appear as a Netscape-specific FORM content element, just like INPUT, SELECT and TEXTAREA. This should be fixed in later printings.

The acnowledgements section was not printed in early printings of the book -- they should be on page XX. This should be fixed in later printings. For completeness, the acknowledgements are available on this Web site.
