WELCOME TO THE REGISTRATION SECTION FOR HEALTH PROMOTION SUMMER SCHOOL

COURSES AND OVERNIGHT ACCOMMODATION WILL BE OFFERED AT BMO INSTITUTE FOR LEARNING.

PLEASE REGISTER WITH CHP FOR COURSE, AND REGISTER WITH IFL FOR ACCOMMODATION (SEE DETAILS BELOW).

Registration for HPSS 2009 is possible by the following means:

A) ON-LINE: Registration on-line may be made by completing on-line form.

Payment may be made by submitting credit card number on on-line form, or by submitting on-line form and arranging for cheque or money order to follow. 

Please make cheque or money order payable to the University of Toronto.

B)FAX: Registration by fax may be made by downloading form (click here) and faxing registration form to: Michelle Rosen, CHP, at 416-971-1365.


Payment may be made submitting credit card number on downloaded-form, or by submitting form and arranging for cheque or money order to follow.

If Submitting a cheque or money order please make cheque payable to the University of Toronto.


C) CANADA POST: Registration by mail may be made by downloading form (click here) and sending completed registration form to:

NB: When completing the registration form, please write legibly and respond to all questions, to avoid delay.

HPSS 2009 FEES: 

HPSS 2009 Full Registration: $ 525

Fees include the following: 

  • full program (July 6-8, 2009)
  • course materials
  • lunches and snacks

Preschool (July 7, 2009): Additional cost

Preschool A: $ 50 (includes course and materials)

Preschool B: $ 100 (includes courses and materials and snacks)

Preschool C: $ 100 (includes courses and materials and snacks)

Registration Deadlines: 

STANDARD REGISTRATION (as of May 10th ): $525
STUDENT RATE : $300 (ONLY FULL-TIME STUDENTS IN ACCREDITED CANADIAN POST-SECONDARY INSTITUTION ARE ELIGIBLE. PROOF OF FULL-TIME STATUS IS REQUIRED UPON REGISTRATION AT HPSS.)
LAST DATE TO REGISTER: June 26, 2009

Cancellations and Refunds:

Up to and including May 23, 2009: cancellations will be fully refunded less a $50 administration fee.
From May 24 to June 6, 2009: 50% of the registration fee will be refunded.
As of June 7, 2009: no refunds for cancellations.

Cancellation requests must be received in writing prior to these deadlines.

Cancellation of accommodation reservations operate on a different schedule and must be arranged directly by participant with the facility hosting HPSS 2009.


REGISTRATION NOTES

(i) Preschool Sessions
Preschool sessions are optional. There is an additional cost for these workshops.
Note: you can attend Preschool only, without attending the rest of the Summer School.

(ii) Series
Participants must select one Series only.
Also: All are welcome and encouraged to attend the First Peoples-specific series (1). Series 2 is offered only in French.
NB Francophone participants (must live or work in Ontario) receiving subsidies must select and attend Series 2.

(iii) Toolbox Sessions
ALL Participants will select two Toolbox sessions (one in Session A and one in Session B) when these are confirmed. (All registrants will be contacted by CHP in March 2009.)
Also: All are welcome and encouraged to attend the First Peoples-specific workshops.

(iv) Personal Development Workshops
Participants must pre-register for Personal Development workshops when these are confirmed. (All registrants will be contacted by CHP in March 2009.)

Also: All are welcome and encouraged to attend the First Peoples-specific workshops..

(v) French-language sessions
Participants must indicate on the registration form if they are attending French-language sessions.

Subsidies for Francophones (living in Ontario)
Participants must indicate on the registration form if they are applying for a subsidy.
NB Please contact Coordinator immediately to be placed on list. First registered, first served.

2. REGISTRATION FOR OVERNIGHT ACCOMMODATION (BMO INSTITUTE FOR LEARNING)

Please note: REGISTRATION FOR OVERNIGHT ACCOMMODATION MUST BE SENT DIRECTLY TO THE FACILITY (BM0 IFL) WHICH WILL HANDLE ALL REGISTRATIONS FOR ACCOMMODATION. PLEASE COMPLETE THIS FORM AND SEND TO THE IFL BY EMAIL (IFL.RESERVATIONS@BMO.COM), FAX (416-490-4443) OR BY CANADA POST (3550 Pharmacy Avenue, TO, M1W 3Z3).

Onsite guestrooms at the IFL are Single occupancy. If you require alternate arrangements,please contact our Reservations Department (416) 490-4300, and they will be happy to make alternate arrangements for you.

COST: $160 PER NIGHT + 10 % GST AND PST (INCLUDES COMFORTABLE SINGLE ROOM, DELICIOUS SUPPER AND BREAKFAST, PARKING, AND RECREATIONAL FACILITIES).
