While the research initiatives undertaken under
the auspices of the Chair in “Health Care Settings and Canadians”
are diverse, two broad lines of research have been defined.
Descriptive Research
and Theories of Explanation
This approach integrates perspectives from the health, clinical,
and social sciences in order to describe and explain variations
in health practices, health management decisions, and health policy
making. This approach to research is based on the premise that
to know what current practices are, to explore why they are what
they are, and to develop valid and reliable predictive estimates
of human and institutional behaviour are all of general and lasting
value, especially as precursors to theory and hypothesis generation.
Applied
Evaluative Research
This approach focuses on applied evaluative and policy research
pertaining to efficient, effective, and equitable health care
services in settings where health care is sought, delivered, and
received. This approach is based on the premise that the development
and application of theories of explanation and new methods of
analyses are essential to guide empirical research and to assist
in the informed interpretation of evaluative findings. Hence,
the evaluative research undertaken will be theory-based and will
include the development and application of new methods of inquiry.

2009 Funded Research Grants
Predictors of Place of Death and Family Caregiver Burden Across the Home-Based Palliative Care Trajectory
(funded Partnerships for Health Systems Improvement - CIHR)
Publicly-financed home-based palliative programs have been established to provide community multidisciplinary care, including practical and emotional support, to individuals at home. Despite patients' preferences for home death and the establishment of specialized, home-based programs, which aim to support death at home, many individuals with cancer continue to die in institutional settings. Understanding the determinants of place of death and the associated burden on families may inform the targeting of services to those patients for whom interventions to support home death may be more useful and effective. Moreover, acquiring economic evidence pertinent to place of death is critical to highlight differences in resource utilization between institutional deaths and home deaths. The purposes of this study are to:1)to assess the economic, demographic, clinical, and psychosocial factors that predict whether cancer patients who are referred to a home-based palliative care program die at home or in an institution; 2)to assess the economic, demographic, clinical, and psychosocial factors that predict caregivers' burden, across sites of death; and 3)to estimate the societal costs associated with the entire episode of palliative care, including resource costs incurred by patients & their family/friends during hospitalizations, until the time of death. Family Caregivers will be recruited from two programs: Temmy Latner Centre for Palliative Care in Toronto and the Hamilton Niagara Haldimand Brant LHIN Community Palliative Care Teams. Family caregivers will be asked to provide information on health services resource utilization, functional status, symptom severity & psychosocial burden. All other required data will be obtained from existing regional databases maintained by the palliative care programs. It is hoped that the study findings will assist in the formulation of health planning & resource allocation initiatives pertaining to palliative care.
Health Care, Technology and Place (HCTP) II : A CIHR Strategic Training Initiative
The Health Care, Technology & Place (HCTP) Program is the only graduate training program that tries to understand 21st century health care by examining the way that technologies, societies & space interact. HCTP provides an integrated, collaborative & interdisciplinary approach to research training. In its first 6 years, HCTP1 generated a group of scholars who built research capacity through scholarship that links many academic disciplines & examines the complex linkages between people, places, technologies & work in modern health care. While HCTP1 focused on the setting (or places) where health care is sought, delivered or received, HCTP2 looks also at technology since increasingly, people who participate in care (patients, providers) & the technologies they use can be separated in space or time. In looking at technologies, HCTP continues to examine the people, places & work practices through which technologies are produced, understood, & have their effects. Also, HCTP2 works to encourage a broader approach to the highly practical & policy relevant domain of health technology assessment (HTA) in order to capture the wide-ranging & context-specific features of technologies in health systems & everyday life. While funded in the first wave of CIHR training programs, HCTP has evolved. This application offers evidence of innovation & progress as: HCTP2 has the same name but an enhanced emphasis on the technological mediation of care; new mentors add disciplinary depth & complementary & distinguished records to the HCTP1 mentor team; the HCTP2 curriculum structure involves innovations in design & content to ensure greater integration & core shared content; knowledge translation across disciplines is augmented by knowledge transfer & exchange (KTE) with the fields of practice & policy; & research training has been transformed & supplemented through research placement opportunities with decision maker organizations at the local, national & international levels.

"Bringing International Experience Back Home"
- Aruna Dhara (India)
Aruna Dhara, MD, received funds to pursue a research project in India in the summer of 2006. The following is an outline of her project entitled "Evaluation of Health Insurance and Preventative Health Care in Rural India"
" I am very grateful for the CHSRF/CIHR Chair funding provided to me for my research in India. I am interested in understanding the effects of microhealth insurance and health education on health and economic outcomes through a randomized control trial in rural India. Dr. Coyte's assistance in setting up this project, which occured this past summer, has been invaluable to me. Without the financial support of the CHSRF/CIHR I would have been entirely unable to pursue this project, and I am incredibly grateful to him for his mentorship. His support of students interested in health services research is greatly appreciated."
We are conducting an "Evaluation of Health Insurance and Preventative Health Care in Rural India", which will take place in Karnataka, India. The intervention is expected to begin in March of 2007 and will be completed March 2008. We will offer low cost health insurance in 100 villages and free health education in 100 villages, cross overlapping them so that there will be four different study groups in total; two with either of health education or insurance, one with both interventions and a final control group with no intervention. To evaluate the effects of the interventions, we will conduct surveys of 30 households in each village. These interviews will have two parts: part one will inquire about economic and health status of household members. Part two involves taking simple anthropometric measures; height, weight, blood glucose, and hemoglobin. The surveys will be conducted at baseline, prior to intervention, and at 1 year after the intervention.
-Meghan McMahon (Paris)
As part of her MSc program, Meghan McMahon completed a 3-month internship in the Health Division at the Organization for Economic Co-operation and Development (OECD) in Paris, France, where she worked with a team examining pharmaceutical pricing policies and innovation. Meghan gained valuable insight into the importance of balancing competing (political and economic) priorities as well as practical experience in a non-academic research environment.
" I feel extremely fortunate to have had this opportunity to work with a fantastic team of researchers and policy experts in the Health Division at the OECD. This experience enriched my master's training as no other would, and I am sincerely grateful for the ongoing encouragement and support of my MSc supervisor, Dr. Peter Coyte, who made this internship opportunity a reality."
-Alexandra Mihailovic (Uganda)
Alexandra Mihailovic, MD, is pursuing a PhD in Clinical Epidemiology supervised by Dr. Peter Coyte and Dr. David Urbach. Her clinical interests are in the field of trauma and international surgery, which aligns with her thesis topic of access to care after pediatric injury in sub-Saharan Africa. This topic is in response to a growing concern of the enormous burden of disablity and early mortality caused by both intentional and unintentional injury in low income countries. This research has taken Alexandra to Uganda where she spent 2 years collecting data on child injuries and working clinically in the war ravaged northern regions of the country. In response to her research findings, she started a child injury prevention program that saw a 15 fold decrease in pedestrian injuries in the research population.
"My experience conducting this research in Uganda was invaluable to my future career in bringing epidemiological and public health methods addressing surgical and injury crises in the developing world. Having the opportunity to work first hand with local researchers and clinicians, to experience the challenges of conducting research and search for their solutions in collaboration with the research teams in these settings and to witness the obstacles to public health programs and policies and the actualy health priorities dictated by the public have allowed me to embark on my field of study and clinical interest with a solid understanding of the realities and needs."
